Once the correct Requested Procedure has been identified from the list, the 4-digit Procedure Code may be selected from the full index list for the Movement Type

Each Requested Procedure provides a list of the allowable 4-digit codes.

If the 4-digit Procedure Code combination is not shown in the index list it is not permitted.
