Checklist for Declaring AI codes

Once the full set of AI codes are identified, the AI statements should be completed using the following steps:

  • Any union codes should be declared first

  • Declare the AI 5-digit code followed by the required additional information text

  • Use the Appendix 1 completion instructions for the chosen Procedure Code, where given.

  • Use the Appendix 2 completion instructions for the chosen Additional Procedure Code, where given

  • Use the Appendix 4 completion instructions where the code is not covered by Appendix 1 or 2

  • Where the AI code requires specific text to be used, it should be completed as stated in the guidance
