How to identify an unsuccessful attempt to make a claim
Examples of an unsuccessful attempt to make a claim are
Claims made after expiry of the time limit
Declaration on the claim form not signed
Declaration signed but not by the claimant or other suitable signatory where appropriate
Not answering relevant questions on the claim form
Not supplying all the other information or documents requested in the claim form or notes
