Step 1
Fully complete part 1 of the form CA8288 in the following way, go to Step 2.
Step 2
Complete the ‘NTC trace request’ section as follows
place a tick in the ‘MU286101’ field
leave the ‘Unique ID (STU Use Only)’ field blank
enter today’s date in the ‘Date’ field
leave the ‘Time’ field blank
enter your first name and surname in the ‘Original operator’ field
in the ‘Office address’ field, enter (This content has been withheld because of exemptions in the Freedom of Information Act 2000)
leave the ‘Office Code’ field blank
enter your full work phone number, including STD code in the ‘Telephone’ field
enter your full work fax number, including STD code in the ‘Fax’ field
Note: If you’re dealing with a NINO request, don’t select the ‘Supervisor’ checkbox. Your line manager will complete this.
Note: If you’re dealing with a NONO request, select the ‘Supervisor’ checkbox.
go to Step 3.
Step 3
Complete the ‘Details provided by claimant’ section a follows
enter the title in the ‘Title’ field
leave the ‘NI Number’ field blank
enter the customer’s surname in the ‘Surname’ field
enter the customer’s first names in the ‘First name(s)’ field
Note: If the surname or first names are too long to fit in the respective fields then enter as much as you can in those fields and the remainder in the ‘Further information’ field.
enter the full current address, including postcode if known in the ‘Address current’ field
enter all known previous names in the ‘Previous names(s)’ field
enter the last known full previous address, including postcode in the ‘Previous address’ field
go to Step 4.
Step 4
Complete the ‘Details found on NTC trace - if different’ section as follows
enter the title in the ‘Title’ field
leave the ‘NI Number’ field blank
enter the customer’s surname in the ‘Surname’ field
enter the customer’s first names in the ‘First name(s)’ field
Note: If the surname or first names are too long to fit in the respective fields then enter as much as you can in those fields and the remainder in the ‘Further information’ field.
enter the full current address, including postcode if known in the ‘Address current’ field
go to Step 5.
Step 5
Complete the ‘Additional Information’ section as follows
enter the partner’s NINO in the ‘Partner NINO’ field
Note: Leave blank where the customer is claiming under Derived Rights and their partner (if they have one) does not have a NINO.
place a tick in ‘True’ field for ‘Claimant stated that they have never had a NINO’
Note: You should’ve already confirmed with the customer that they’ve never had a NINO.
go to Step 6.
Step 6
Complete the ‘Further information’ field as follows
in the ‘Further information’ field state the reason the customer doesn’t have a NINO or NONO and also the remainder of the first name or surname which couldn’t be entered in the respective fields because they contained too many characters.
If you’re requesting a NONO
email the CA8288 to your line manager for them to complete the ‘Supervisor’ checkbox
forward the completed CA8288 by email to the designated STU mailbox (This content has been withheld because of exemptions in the Freedom of Information Act 2000) . You’ve now completed the CA8288.
If you’re requesting a NINO
send the completed CA8288 form as an attachment by email to your line manager for checking together with the casepapers
go to Step 7.
Step 7
Your line manager will check the case.
If they’re satisfied that the NINO referral is correct
they’ll select the ‘Supervisor’ checkbox
they’ll forward the completed CA8288 by email to the designated STU mailbox (This content has been withheld because of exemptions in the Freedom of Information Act 2000)
they’ll pass the casepapers back to you. You’ve now completed the CA8288.
If they decide that further information is required, they’ll return the referral back to you together with the casepapers with clear instructions on what to do next.
